Meds@HOME is a digital intervention designed to improve medication safety for children with medical complexity (CMC) through better coordination among caregivers. The application aims to address the lack of tools supporting coordinated medication delivery, ensuring that both primary and secondary caregivers can effectively manage medication administration at home.

Problem Statement
Children with medical complexity (CMC) often require intricate medication regimens that are difficult to manage, especially when multiple caregivers are involved. There are currently no easily accessible tools to support coordinated medication delivery in the home setting. This results in potential medication errors, missed doses, and lack of proper communication among caregivers.
MedS@HOME solves this problem by providing a digital platform that:
- Helps caregivers track medication administration in real-time.
- Coordinates efforts between primary and secondary caregivers.
- Enhances communication with clinical experts to ensure safe and timely medication management.
Goals

Target Audience

Key Features
- Medication Tracking: Caregivers can log each medication administered to the child, including dosage, time, and any special instructions. Alerts notify caregivers of missed doses or upcoming medication schedules.
- Caregiver Coordination: The app enables multiple caregivers to access and update medication schedules in real-time, ensuring coordination between all parties.
- Clinical Integration: Clinical experts can access the medication log, providing remote guidance and ensuring that medication administration aligns with best practices.
- User Profiles: Each caregiver (primary, secondary, expert) has a customized profile with access levels tailored to their role. Primary caregivers can grant or restrict access to other users.
- Medication Reminders and Alerts: Automated notifications remind caregivers when it's time to administer medications and when a scheduled dose is missed.
- Reports & Analytics: Data is visualized through easy-to-read reports that caregivers and healthcare providers can review to assess medication adherence and overall health status.
Results & Impact
By the completion of the MedS@HOME app, key outcomes included:
- Improved Medication Adherence: Caregivers were able to track medications more reliably, leading to fewer missed doses.
- Increased Coordination: Both primary and secondary caregivers were able to coordinate their efforts effectively, reducing confusion and improving the overall caregiving experience.
- Enhanced Medication Safety: With real-time updates and clinical integration, medication errors were significantly reduced.
- The MedS@HOME app has received positive feedback from caregivers and clinical experts, highlighting its potential to improve medication safety for children with medical complexity across various caregiving networks.
